Your 40s are usually when the jawline softens and the mid-cheek starts to drop. A pairing of ultrasound (HIFU) and radiofrequency (RF) lifting tends to work best here, planned around a steady yearly rhythm.

In your 40s, the most effective non-surgical lifting is often a combination: HIFU such as Ultherapy PRIME to support the deeper layer, plus RF such as Thermage FLX or XERF to tighten the skin. The jawline and mid-face are usually treated first. One main combined session a year, with lighter skin care in between, suits most people in this decade.
HIFU and RF work better together in the 40s because sagging now happens at two levels at once. The deeper support layer (the SMAS, the same layer a surgeon lifts in a facelift) starts to loosen, and the skin above it loses collagen and elasticity.
Micro-focused ultrasound, as in Ultherapy PRIME, places tiny heat points in deeper tissue to support lift along the jaw and brow.
Radiofrequency, as in Thermage FLX or XERF, heats the dermis to tighten and renew collagen, improving crepiness and firmness.
Using only one usually leaves part of the problem untreated. Firm skin over a sliding support layer still looks heavy, and a lifted support layer under loose skin still looks crepey.
For most people in their 40s, the jawline comes first, because a blurred jaw line is the change others notice soonest. The mid-face is a close second, as early cheek descent deepens the nasolabial folds and makes the face look tired.
The doctor maps energy to these areas rather than treating the whole face evenly. For a jaw-focused plan, see our page on jawline lifting.
These are the combinations most often considered for patients in their 40s at Gangnam Celliting Clinic. The doctor adjusts devices and energy to your face.
| Combination | Deeper layer | Skin layer | Best for |
|---|---|---|---|
| Ultherapy PRIME + Thermage FLX | HIFU | Monopolar RF | Jowls with crepey skin |
| Ultherapy PRIME + XERF | HIFU | Multi-frequency RF | Laxity plus pores and texture |
| One-Day Combined Face Lifting | HIFU | RF plus Olla Wave and Fortra | Full-face lift in one visit |
| Lifting + skin booster | HIFU or RF | Booster for quality | Laxity with dry, dull skin |
The One-Day Combined Face Lifting brings Ultherapy PRIME, Thermage FLX, Olla Wave and Fortra together in about 60 minutes to 2 hours, with minimal downtime. Details of the RF part are on our Thermage FLX page.
A simple yearly rhythm keeps results steady without over-treating. Collagen remodelling continues for several months after a combined session, so frequent repeat lifting adds little.
Suitability is a medical decision made by your doctor after an in-person consultation and review of your health history.

Numbing cream, AirNox nitrous-oxygen analgesia or sleep sedation may be offered for combined lifting.
HIFU is usually followed by RF in the same visit, with mild redness that settles quickly.
Written aftercare in your language and a free 3-month follow-up let you report progress remotely.

No serious complication since 2007. FDA/KFDA-cleared devices and verifiable, genuine consumables.
Monitored sleep sedation, AirNox analgesia, the X-injector or local anesthesia, matched to the procedure.
Because combined lifting has minimal downtime, two to three days in Gangnam, Seoul is usually enough. Many overseas patients book their main yearly session around a regular trip to Korea. Gangnam Celliting Clinic is about a 4-minute walk from Gangnam Station Exit 12, and the doctors explain each step in English, Chinese, Japanese or Vietnamese. Avoid saunas and intense exercise for a day or two afterwards.

Usually neither alone. HIFU reaches the deeper support layer, while RF tightens the skin itself. In the 40s both layers are typically involved, which is why the doctors often combine them in one session.
Early, mild jowls often improve with combined HIFU and RF for suitable patients. Results are more modest than surgery and develop gradually. If jowls are heavy or skin is very loose, the doctor will discuss whether surgery would be more effective.
Some areas feel hot or sharp, especially along the jaw. The clinic matches comfort options to the procedure, including numbing cream, AirNox analgesia and, where appropriate, monitored sleep sedation. Most people find it manageable.
Some tightening may be visible soon after, but the main improvement usually builds over about two to three months as new collagen forms. Plan photos and events with that timeline in mind.
Usually not. For most people in their 40s, one main session a year is enough, with skin-quality care in between if needed. Repeating too often may not add benefit and can thin facial fat in slim faces.
